• Company name
  
• Contact person
  
• Address
• Phone (office, direct, cell)
• Email
• Website

and information for each product:

• Product (as listed on 205.606).
• Product Name (as it appears on your label).
• Product Description.
• Link to web page about the product.
• Unit sizes of commercially available packaging. (For example, “1 quart bottle”, “1 Kg bag”, “50# bag”, “1000 Kg tote”, “custom”, etc.
• Name of Certifier.

Please attach a copy of your current organic certificate for this product.
